Bring the water to a simmer and cook for 2 1/2 to 3 hours, refilling the water every 15-20 minutes or so to keep it at the same level you started with. Over 2-3 hours, the sweetened condensed milk will transform into a golden caramel. The longer it cooks, the deeper the caramel color and thicker the resulting caramel.
